When you want clarity about symptoms and a path forward for support
Many adults seek Mental Health, ADHD, or Women’s Health Assessments because they want clarity about their symptoms and a clear path forward for support. Understanding what is contributing to challenges with attention, mood, daily functioning, or specific women’s health concerns can help identify effective strategies and resources.
You might be asking yourself:
- Why do I find it so hard to concentrate or stay organized in my daily life?
- Is this ADHD, mental health, or something else?
- Why do I feel anxious, depressed, restless, or overwhelmed most of the time?
- How do I find answers about changes in my menstrual cycle, hormonal balance, or reproductive health?
Symptoms can sometimes overlap or influence each other, making it hard to understand what’s really going on. An assessment provides insight into your unique needs and guides next steps for treatment or support, empowering you to make informed decisions about your well-being.
At Colibri NeuroWellness, we take time to listen to your experiences, review your medical history, and use screening tools or lab tests to get a clearer picture. Through this, we can help identify whether your symptoms are related to hormonal changes, mental health conditions, ADHD, or a combination of factors.
- Stress, overwhelm, burnout, or difficulty coping with daily life
- Symptoms related to perimenopause, menopause, or post-menopause
- Brain fog, forgetfulness, difficulty coping, or trouble staying focused
- Persistent feelings of sadness, hopelessness, or emptiness
- Changes in vaginal discharge, odor, or itching
- Questions about sexual health, STI testing, or preventative screening
- Changes in appetite, weight, or sleep patterns
- Concerns about fertility, contraception, or sexual health
- Withdrawing from friends, family, or social activities
- Challenges with organizing tasks, managing time, or meeting deadlines
- Difficulty enjoying activities you used to like
- Severe menstrual cramps, pelvic pain, or pain during intercourse
- Changes in mood or energy around your menstrual cycle or menopause
- Feeling easily overwhelmed, irritable, or angry
- Ongoing anxiety, excessive worry, or panic attacks
- Trouble concentrating, making decisions, or remembering things
- Irregular, missed, or unusually heavy periods
- Frequently starting new projects but struggling to finish them
Our nurse practitioner services can help you see the whole picture, then move forward with clearer next steps.

Nurse Practitioner Services We Offer
Personalized Solutions
Our nurse practitioner offers assessment, consultation, treatment planning, and follow-up care for adults who want a clearer understanding of their symptoms.
At Colibri NeuroWellness, our NP services focus on three main areas:

Women’s Health Assessments, Consultations & Follow-Ups
Learn more
Women’s Health support with a Nurse Practitioner (NP) can help you understand changes related to perimenopause, menopause, post-menopause, sexual health, sleep, mood, and daily functioning.
This kind of care can help with hot flashes, night sweats, insomnia, mood changes, anxiety, or irritability, brain fog and memory concerns, and genitourinary syndrome of menopause (including vaginal dryness, burning, itching, or painful intercourse).
It can also support urinary urgency, frequency, or incontinence, STI testing and sexual health concerns, preventative screening recommendations, and considerations around bone, metabolic, and cardiovascular health.

Mental Health Assessments, Consultations & Follow-Ups
Learn more
A Nurse Practitioner (NP) can help assess Mental Health symptoms, rule out contributing medical factors, and support you with a plan that may include therapy recommendations, medication consultation, lifestyle strategies, diagnostic clarification, or follow-up care.
This kind of support can be helpful if you’re navigating anxiety, depression, mood changes, or emotional overwhelm. It also extends to stress and burnout, poor sleep and fatigue, physical symptoms tied to mental health (such as headaches, muscle tension, nausea, or a racing heart), and questions around medication or treatment planning.

Adult ADHD Assessments, Consultations & Follow-Ups
Learn more
Adult ADHD can be confusing, especially when you’ve spent years blaming yourself for things that may be connected to executive functioning.
An Adult ADHD assessment can help you better understand how your brain works, whether ADHD is part of the picture, and what support may help.
The evaluation process includes a conversation about your current concerns, a review of personal, medical, and developmental history, ADHD-focused screening questionnaires, and consideration of anxiety, depression, sleep, stress, hormones, and other possible contributing factors. Based on this information, you will receive feedback, a diagnosis when applicable, and a plan for next steps.
Understanding what your body and mind need can bring relief, direction, and real momentum toward feeling better.

Why Work With Us?
NP Services include health assessments, diagnosis and management of mental health conditions, ADHD evaluation and treatment, reproductive and gynecological care, and medication management.
Whole-person care
Your symptoms are all connected: Sleep can affect your mood, hormones can influence your focus, your mental health can impact your physical well-being, and ADHD can also affect your relationships, work, energy, and self-confidence. We take all of these factors into account to understand your experience and provide the best care for you.
Clear answers, plain language
You deserve care that makes sense. We explain what we are looking at, what may be contributing to your symptoms, and what your next steps could be.
Support that fits real life
Your plan may include lifestyle strategies, medication consultation, follow-up care, therapy recommendations, screening, or referrals. We focus on practical steps that align with your schedule, lifestyle, and values.
Care for complex overlap
Perimenopause can look like anxiety. ADHD can look like burnout. Insomnia can make everything worse. Mental health concerns can show up in the body. We help you sort through the overlap and make sense of what needs support first.
Collaborative support
When helpful, and with your consent, we can collaborate with other members of your care team so you aren’t left piecing everything together on your own.
Guidance beyond the first appointment
An assessment is the beginning, and then we develop a follow-up care plan to help you adjust the plan, ask questions, monitor changes, and feel supported as you move forward.

Our Approach
Begin With Understanding
We start with a brief screening call or email with our administrative team to choose the service that fits, whether that’s a Women’s Health, Mental Health, or an Adult ADHD Assessment.
You will be emailed an Intake Form to complete ahead of time so our Nurse Practitioner can use your appointment well.
If you would like to skip this screening, the Initial Appointment can be scheduled through our Nurse Practitioner’s online booking page, and the Intake Form will be emailed to you to complete.
Initial Appointment or Consultation
This is a secure online meeting where you can share your concerns and ask questions in a comfortable, confidential setting.
During this time, your Nurse Practitioner (NP) will listen and learn about your health background and current symptoms, so we can understand your unique needs and experiences. This may also involve discussing your mood, attention, behaviour, and how these concerns affect various aspects of your life, such as work, relationships, and self-care.
Targeted Screening, Imaging, and Bloodwork
Your Nurse Practitioner will order any necessary bloodwork or imaging to investigate further into Women’s Health concerns.
For Mental Health or ADHD Assessments, this includes the use of standardized rating scales and screening tools to evaluate symptoms, severity, and impact on daily functioning.
Follow-Up Appointments
During your Follow-Up appointment, you and your Nurse Practitioner will review your results together, discuss any diagnoses, and explore treatment options, including lifestyle changes, medications, and referrals to specialists or other healthcare providers as needed.
Additional Follow-Up Care
Additional Follow-Up Appointments provide an opportunity to reassess your progress, address any new or changing symptoms, and make adjustments to your medication or treatment plan as needed.
Your Nurse Practitioner will work with you to ensure your care remains aligned with your evolving needs and goals, offering guidance and resources to promote long-term health and well-being.

Questions About Our NP Services
Who can see the NP?
Adults 18 and older with concerns related to women’s health, mental health, or focus and attention. Common reasons people book include perimenopause symptoms, mood changes, ongoing insomnia, sexual health questions, and curiosity about adult ADHD. If you’re not sure which service is the right fit, our team will help you decide during the screening call.
Can the NP diagnose and prescribe?
Yes. Nurse Practitioners in Ontario can diagnose conditions, order tests, prescribe medications, including hormone therapy and ADHD medications where clinically appropriate, and provide ongoing follow-up care.
Do I need a referral?
No. You can book directly. With your consent, we are happy to communicate with your family doctor or other providers so your care stays coordinated.
Is this the same as therapy?
No. Our NP provides medical assessment, consultation, and ongoing care. Some appointments include a brief supportive conversation, but for psychotherapy, Heather can refer you to one of our psychologists or counsellors, or coordinate with a therapist you already see.
How is an NP-led ADHD assessment different from a psychoeducational assessment?
Our Nurse Practitioner-led Adult ADHD Assessment is a focused evaluation designed to help adults get clear answers about ADHD, talk through treatment options, and discuss whether medication might be helpful.
If you need a more detailed evaluation, such as formal documentation for school or work accommodations, or if you think a learning disability might be involved, our Psychologists can provide a comprehensive Psychoeducational or Neuropsychological Assessment.
If you’re not sure which assessment is right for you, our team is here to guide you and help you choose the best option for your needs.
What exactly are perimenopause and menopause, and how do I know if I'm in it?
Perimenopause is the transition leading up to menopause, when hormones like estrogen and progesterone start to fluctuate, and your cycle becomes less predictable. Menopause itself is reached once it’s been twelve months since your last period, and the years after are called post-menopause.
Common signs along the way include cycle changes, hot flashes, night sweats, mood shifts, sleep problems, and vaginal dryness. Because these symptoms can overlap with stress, thyroid issues, or other conditions, our NP will take a full history, order targeted bloodwork if it’s helpful, and help you make sense of what’s hormonal and what might be something else.
What does an assessment with an NP involve?
Assessments with a Nurse Practitioner (NP) include a 60-minute Intake Appointment and two 30-minute Follow-Up Appointments. Additional Follow-Up Appointments can be scheduled afterward on an as-needed basis, and may include medication re-assessment or renewal.
(1) Intake Appointment: Your NP will ask you questions about your current concerns, your symptom history from childhood to now, and looking at how these symptoms are showing up in your daily life. Your NP will also consider other things that can look like ADHD, including anxiety, sleep difficulties, perimenopause, or chronic stress, so the picture stays accurate.
Providing Elementary School report cards with teacher comments, or having a family member provide collateral information is optional but not required.
What happens after my Initial Appointment?
After your Initial Appointment, your Nurse Practition may ask you to complete screening questionnaires or tests to investigate Mental Health or ADHD concerns. If you have Women’s Health concerns, she may order medical imaging or bloodwork for you to complete.
During a virtual Follow-Up Appointment, your Nurse Practitioner will go over your results, communicate any diagnosis (or diagnoses) talk through your treatment options, and discuss receiving a brief report or diagnostic letter if appropriate.
Depending on the results, your treatment plan might include lifestyle suggestions, medication, additional testing, ongoing follow-up appointments, or a referral to another provider on our team or elsewhere. Whether you came in with women’s health questions, a mental health assessment, or curiosity about adult ADHD, we’ll help you find clarity around what’s going on and practical next steps you can take.
How many appointments will I need, and how do I know it's working?
Assessments include three appointments: the 60 minute Initial Appointment and two 30-minute Follow-Up Appointments. However, there’s no set number if you would like to schedule additional Follow-Up Appointments, such as discussing a medication adjustment or new symptoms.
Early on, you and your NP will set goals that matter to you, like sleeping through the night, less brain fog at work, or fewer hot flashes, and revisit them at follow-ups so progress is easy to see. Signs things are heading in the right direction often include better sleep, steadier mood, easing insomnia, and feeling more like yourself in daily life. If things stall, your NP can adjust the plan, change medications, or bring in additional support so your care stays flexible.
Are NP appointments in-person as well as virtual/online?
No. All Nurse Practitioner appointments are offered exclusively through secure virtual visits, available to clients anywhere in Ontario. This makes it easier to access care if you live outside the region, have mobility challenges, or have work hours that don’t fit a typical clinic schedule. If you require medical imaging, bloodwork, or any in-person testing/screening, these will be arranged at a lab or clinic most convenient for you to visit.
If a referral is made to see another healthcare provider at Colibri NeuroWellness or to an external specialist for ongoing care, in-person visits will be coordinated as appropriate.
